Bipolar Forceps Types: European vs US Two Pin, Tips, Irrigation and Non Stick

How to choose a bipolar forceps by connector, shaft, length and tip, when irrigation and non stick tips help, and the patterns we make.

A bipolar forceps coagulates tissue held between its two tips: current passes from one tip to the other, not through the patient's body, so the effect stays where you grip. Choose it in four steps. First the connector, European or US two pin, to match your cable and generator. Then the shaft: straight for the surface, bayonet or angled for deep and narrow fields. Then the length for the depth, and the tip width for the vessel size. Irrigating and non stick versions cut down on tissue sticking to the tips.

Where did bipolar forceps come from?

Bipolar coagulation forceps were first presented by the Houston neurosurgeon James Greenwood in 1940, and the configuration is still standard today, according to a review in Frontiers in Surgery. The Texas Medical Center archives credit Greenwood with inventing the forceps and with bipolar electrocoagulation that let surgeons control bleeding while irrigating. Malis later produced the first commercial bipolar coagulation system in the 1960s. Irrigating and non stick tips came later, to deal with the oldest problem of bipolar work: tissue sticking to the tips.

European or US two pin fitting?

The fitting is the connector at the back of the forceps where the cable plugs in. Bipolar generators and cables use different connector standards, and a forceps only works with a cable made for its fitting. We make both European fitting forceps, with a matching European bipolar cable, and US two pin fitting forceps. Check the bipolar socket on your generator and the cable you use before ordering. If you are not sure, send us a photo of the connector and we will confirm.

How do you choose tip, length and features?

  • Tip width. 0.5 mm for the smallest vessels and the finest work, 1 mm for general use, 1.5 to 2 mm for larger vessels and broader coagulation.
  • Length. Short forceps for the surface, 18 to 24 cm for deep cavities.
  • Shape. Bayonet shafts keep your hand below your line of sight under a microscope or in a narrow field. Angled tips reach around structures.
  • Irrigation drips saline at the tip, which cools the tissue and reduces sticking and char. It needs an irrigation source.
  • Non stick tips have a coating that reduces adhesion without needing irrigation.

How should bipolar forceps be cleaned?

Wipe char off the tips during the case with a moist sponge, never by scraping, because scratches make tips stick more. Flush irrigation channels straight after use. Before every use, inspect the shaft insulation for cracks and check that the tips meet evenly: damaged insulation must take the forceps out of use. Never leave instruments in saline, because it causes corrosion. Our instrument care guide gives the full routine, and the CDC guidance on cleaning instruments before sterilization explains why cleaning comes before the autoclave.

Frequently asked questions

What is the difference between European and US bipolar forceps?

The connector. Each fits only a cable made for that connector standard, so match the forceps to your cable and generator.

What tip width should I choose?

0.5 mm for microsurgery and fine vessels, 1 mm for general use, 1.5 to 2 mm for larger vessels.

Why use irrigating bipolar forceps?

Saline at the tip cools the tissue and reduces sticking and char, which helps in delicate fields.

Can bipolar forceps be autoclaved?

Ours can. Check the insulation and tip alignment after each cycle.
