The Barraquer Infant Lid Speculum with a closed wire blade (SA-790-34) is a self-retaining eyelid retractor sized specifically for the neonatal and infant eye. It uses the classic Barraquer spring design — two curved arms joined by a flat tensioned spring, each arm ending in a slim, closed wire loop rather than a solid blade — scaled down to a 4mm blade to suit the smaller palpebral fissure of infants and young children. It is one of the most frequently stocked pediatric ophthalmic instruments because nearly every infant eye procedure, from cataract extraction to exam-under-anesthesia, starts with safe, reliable lid retraction.

Why a Wire Blade for Infant Cases

Adult Barraquer speculums are available with solid machined blades, but for infant work the closed wire blade is usually preferred. A wire loop distributes contact pressure over a rounded, low-mass surface rather than a flat blade edge, which reduces the risk of marking or bruising the thinner, more delicate periorbital skin and conjunctiva of an infant. “Closed” means the wire forms a complete loop rather than an open hook, so there is no exposed wire end that could catch or abrade tissue during insertion, adjustment, or removal — an important safety detail when the patient cannot indicate discomfort and the surgeon is often working at very close range under high magnification.

Design and Mechanism

Like all self-retaining Barraquer specula, this instrument does not need to be held by an assistant once placed. The surgeon gently retracts the upper and lower lids, slides the wire loops just inside the lid margins, and releases; the spring tension in the body holds the loops apart and keeps the lids open for the duration of the procedure. The compact 4mm blade size keeps the overall footprint of the instrument small, which matters in infant surgery where the entire orbit and surrounding working space is proportionally tiny compared with an adult field, and where a bulkier retractor would restrict the surgeon's access or the microscope's view.

Clinical Uses

This speculum is used across the full range of infant and pediatric ophthalmic procedures: congenital cataract surgery, examination under anesthesia for suspected retinoblastoma or glaucoma screening, probing and irrigation for congenital nasolacrimal duct obstruction, strabismus surgery, and minor lid procedures. Because premature and newborn eyes vary so much in size even within the “infant” category, many pediatric ophthalmic units keep both this standard infant speculum and the smaller premature-infant version (SA-790-51) on hand, selecting based on the individual patient's palpebral fissure width rather than age alone.

The finish is polished to resist corrosion through repeated autoclave cycles while keeping the wire loops smooth enough not to create any drag or catch point against lid tissue. Every unit is built to the dimensional tolerances required for consistent spring tension — too little tension and the speculum won't hold retraction reliably; too much, and it risks unnecessary pressure on infant tissue — so tension is balanced specifically for this reduced scale rather than simply being a miniaturized adult spring.

Sterilization and Care

This is a fully reusable, autoclavable surgical instrument. Between cases it should be cleaned promptly to prevent any dried debris at the spring hinge, inspected for bent arms or wire deformation (which will change the retraction tension and blade alignment), and stored in a way that protects the wire loops from being crushed or bent in a crowded tray. Because the tolerances that make an infant speculum work correctly are tighter than on a larger adult instrument, even small deformation is more likely to be clinically noticeable, so infant specula are generally inspected before every single use rather than on a periodic basis.
