Barraquer Infant Lid Speculum Closed Wire Blade

The Barraquer Lid Speculum for premature infants (SA-790-51) is the smallest member of the closed-wire-blade Barraquer family, built for the exceptionally narrow palpebral fissure of a premature or very-low-birth-weight neonate. It follows the same self-retaining spring principle as the standard infant and adult versions, but every dimension — blade length, arm curvature, and overall spread — is scaled down further, making it the instrument of choice in neonatal intensive care and pediatric ophthalmology settings that regularly examine or operate on premature infants, most commonly for retinopathy of prematurity (ROP) screening and treatment.
A standard infant speculum, even at its smallest adult-derived size, can still be too large for a premature eye, where the palpebral fissure may be only a few millimetres wide. Using an oversized speculum on a premature eye risks excessive lid tension, difficulty achieving a secure and atraumatic fit, and a retractor that doesn't sit stably in such a small space. This purpose-built premature size solves that by matching the instrument geometry to the anatomy rather than asking the surgeon to compensate with technique alone — which is particularly important in ROP laser or cryotherapy treatment, where the ophthalmologist needs completely stable, hands-free exposure over what can be a lengthy bedside or NICU procedure.
As with the standard infant version, the blades are closed wire loops rather than solid metal blades. The rounded, low-profile wire minimizes point pressure on extremely delicate premature eyelid and periorbital tissue, and the closed-loop shape (as opposed to an open hook end) eliminates any exposed tip that could catch tissue during placement. Combined with the reduced overall scale, this makes the instrument as gentle as a self-retaining retractor can be while still providing dependable, consistent exposure.
This speculum is a mainstay of ROP screening and treatment, congenital cataract surgery in premature infants, and any bedside or operating-room ophthalmic procedure performed on a neonate below term-equivalent size. Because ROP screening in particular is often performed repeatedly over several weeks as the infant's retinal vascularization is monitored, having a speculum sized correctly for the smallest patients reduces both procedure time and the cumulative tissue stress of repeated examinations.
Built from surgical-grade stainless steel and hand-finished in Sialkot, Pakistan, the instrument is manufactured to the same corrosion-resistance and finish standard as the rest of the Barraquer range, with spring tension calibrated specifically for its reduced size rather than simply scaled proportionally from a larger design. This calibration matters clinically: at this scale, spring behavior does not shrink linearly with size, so a speculum that is only geometrically miniaturized (without re-tuning tension) would either grip too weakly to hold retraction or press with more relative force than intended on very fragile tissue.
The instrument is fully autoclavable and intended for repeated reuse across a standard surgical instrument lifecycle. Given its very fine dimensions, careful handling and storage are essential — the wire loops and arms are more easily bent out of tolerance than a larger adult instrument, and any deformation will directly affect how reliably the speculum retracts such a small lid margin. Routine pre-use inspection for bend, wire fatigue, or loss of spring tension is standard practice wherever this instrument is used.
