Castroviejo Lid Speculum — Overview

The Castroviejo Lid Speculum (SA-790-bs4 family) is a classic self-retaining eyelid retractor available in three blade sizes — 12mm, 14mm, and 16mm — named for Ramon Castroviejo, a pioneering figure in modern corneal and anterior segment surgery. It remains one of the standard reference designs against which many later speculum modifications, including several Barraquer-family variants, are compared.

Design Heritage

The Castroviejo speculum established the now-familiar spring-tensioned, self-retaining principle that underlies most modern eyelid retractors: two curved arms terminating in blades, held apart by a flat tensioned spring body, requiring no assistant to maintain exposure once placed. Its solid blade design and size range were calibrated for general anterior segment and corneal surgery, and the 12/14/16mm size steps remain a commonly stocked range for matching blade width to palpebral fissure size across the adult population.

Sizing

The 12mm size suits narrower palpebral fissures or patients where a more conservative retraction is preferred; 14mm is a general-purpose adult size; 16mm provides wider exposure for procedures needing more working room, such as certain oculoplastic or reconstructive cases. Stocking all three lets a surgical unit match blade width to the individual patient rather than using a single compromise size across all cases.

Clinical Use

This speculum is used broadly across corneal transplantation, cataract surgery, and general anterior segment procedures. Its long-standing, well-characterized design makes it a familiar choice for surgeons trained on classic ophthalmic instrumentation, and it continues to be specified in many standard ophthalmic surgical trays alongside newer speculum designs.
